Please return the last page to Mr. Lawrenson as soon as possible.  It is vitally important that we get these back so that we can keep in close contact with you through the year.

 

If you received a new Raley's Q card, you must register it at raleys.com before we get any funds!  Go to www.raleys.com/qcard to start your registration.  Please note that, in a recent change, you must sign up for a raleys.com account before you can register your card.

One of the major costs for the music department is transporting students to external events - concerts, parades and other performances.  We try to minimize our use of school bus transport by asking parents to transport students in their personal vehicles.

In order to be able to transport students, the Placer Union High School District requires parents to file appropriate paperwork with the school office.  You can download the full driver packet or just the driver form.  You must return the form together with:

• a copy of the details page of your insurance packet showing the required minimum coverage levels
• an abstract of your DMV driving record.  You can obtain this in person from a DMV office, or request it by mail by filling out and returning this form. [If you're going to the DMV office, check the wait times here]

Please either send your completed packet to Mr. Lawrenson, or return it to the school office and notify Mr. Lawrenson so that he has a list of available drivers.  A separate packet is required for each driver.
